If the Department of Business and Professional Regulation certifies your profession or occupation, you must attach a copy of your certification, registration or state license to the application. Any questions, contact 850-487-2252 or www.state.fl.us/dbpr/. If the Department of Agriculture & Consumer Services ( i.e. dance studios, vehicle repair, health clubs) regulates your business, you must submit a registration certificate or letter of exemption.
