Once your form is completed and signed you can mail, drop off, or fax your form to City of Darien, 1702 Plainfield Rd., Darien, IL 60561, Fax No. (630) 852-4709. It takes up to seven business days for the City of Darien to reply to your request. We will notify you by phone when your request is completed.
